实验室衰弱指数对脑未破裂动脉瘤患者手术后并发症的影响

目的探究实验室衰弱指数(FI-Lab)对未破裂动脉瘤手术后并发症的影响。方法对苏州大学附属第一医院2019年1月—2022年1月收治的125例未破裂动脉瘤患者的临床资料进行回顾性分析。根据实验室衰弱指数将患者分为衰弱组(n=29)和非衰弱组(n=96),比较两组患者术后并发症情况。再将患者按照是否出现术后并发症,分为有并发症组(n=28)和无并发症组(n=97),对两组患者的性别、年龄、衰弱、动脉瘤部位、糖尿病、高血压病、手术时长、术中出血量、术后引流量进行统计分析,探究术后并发症的危险因素。结果125例未破裂动脉瘤患者中有28例出现术后并发症,单因素分析结果显示年龄(P=0.029)、衰弱(P=0.001)、糖尿病(P=0.046)、高血压病(P=0.044)、手术时长(P=0.040)是影响术后并发症的重要因素,而性别、动脉瘤部位、术中出血量和术后引流量对术后并发症的影响没有统计学意义(均P>0.05)。多因素Logistic回归分析结果显示衰弱(FI-Lab≥0.3)是未破裂动脉瘤手术患者术后出现并发症的独立危险因素(P=0.021,OR=3.881)。结论衰弱(FI-Lab≥0.3)可能是脑动脉瘤手术患者术后出现并发症的独立危险因素,FI-Lab对预测未破裂动脉瘤手术患者预后有一定的价值。

Objective To explore the influence of laboratory frailty index(FI-Lab) scores on postoperative complications in unruptured aneurysm patients. Methods The clinical data of 125 patients with unruptured aneurysm in the First Affiliated Hospital of Soochow University from January 2019 to January 2022 were analyzed retrospectively. The FI-Lab scores was employed to separate patients into two groups, the frail group(n=29) and the non frail group(n=96), and the postoperative complications of the two were then compared. Subsequently, patients were divided into two groups; the complication group(n=28), and the non-complication group(n=97). Statistical analysis was conducted to examine the sex, age, frailty, aneurysm location, diabetes, hypertension, operation duration, intraoperative bleeding volume, postoperative drainage volume of the patients, in order to investigate the risk factors of postoperative complications. Results A total of 28 out of the 125 people with unruptured aneurysms experienced postoperative complications. Results of univariate analysis showed that age(P=0.029), frailty(P=0.001), diabetes(P=0.046), hypertension(P=0.044), and operation duration(P=0.040) had a significant influence on the occurrence of postoperative complications. There was no statistically significant association between postoperative complications and gender, aneurysm site, intraoperative bleeding volume, or postoperative drainage volume(P>0.05). The logistic regression analysis indicated that frailty(FI-Lab≥0.3) was a significant independent risk factor for postoperative complications in unruptured aneurysm surgery patients(P=0.021,OR=3.881). Conclusions FI-Lab score ≥ 0.3 has been suggested to be a potential indicator of postoperative complications in unruptured aneurysm surgery patients. Additionally, FI-Lab scores may be a useful tool in forecasting the prognosis of those who have undergone unruptured aneurysm surgery.
