目的探讨外伤性癫痫患者miR-7和凋亡分子与脑电图(EEG)放电指数及EEG异常的关系。方法选取2019年1月—2021年1月海安市人民医院治疗的外伤性癫痫患者68例。根据EEG检测结果,其中EEG正常患者11例(EEG正常组),EEG界线性患者20例(界线性组),EEG慢波异常患者21例(慢波异常组),EEG癫痫样放电患者16例(癫痫样放电组),采用2-ΔΔCt法计算血浆微小RNA-7(miR-7)相对表达量,采用酶联免疫试剂盒(ELISA)测定凋亡分子B淋巴细胞瘤-2(Bcl-2)、Bax、Caspase-3的含量。采用脑电图仪进行EEG检查,并计算EEG放电指数。正态分布的资料相关性采用Pearson相关性分析和多元线性回归分析。结果各组miR-7、Bcl-2、Bax、Caspase-3水平比较存在统计学差异(P<0.05),且随着脑电图异常程度的升高,miR-7、Bcl-2水平随之下降,Bax、Caspase-3水平随之升高,EEG放电指数随之增加。Pearson相关性分析结果显示,外伤性癫痫患者miR-7、Bcl-2、Bax、Caspase-3水平均与EEG放电指数存在相关性(P<0.05);多元线性回归分析结果显示,miR-7、Bcl-2、Bax、半胱氨酸蛋白酶-3(caspase-3)水平与脑电图异常均存在相关性(P<0.05)。结论外伤性癫痫患者miR-7和凋亡分子与EEG放电指数及脑电图异常存在相关性,应进行早期EEG检查,并结合患者的病史、病程及miR-7和凋亡分子检测结果,判断其预后及制定科学合理的治疗方案。
Objective To investigate the relationship between miR-7, apoptosis molecules and electroencephalogram(EEG) discharge index in patients with traumatic epilepsy and EEG abnormalities. Methods 68 patients with traumatic epilepsy treated in Haian People’s Hospital from January 2019 to January 2021 were selected. According to the EEG test results, there were 11 with normal EEG(EEG normal group), 20 with EEG boundary linearity(boundary linearity group), 21 with abnormal EEG slow wave(slow wave abnormal group) and 16 with epileptic discharge(epileptic discharge group). The relative expression of plasma microrna-7(miR-7) was calculated by 2-ΔΔCt, and the contents of apoptotic molecules Bcl-2, Bax and caspase-3 were measured by enzyme-linked immunosorbent assay(ELISA). EEG was examined by electroencephalograph, and EEG discharge index was calculated. The data correlation of normal distribution was analyzed by Pearson correlation analysis and multiple linear regression analysis. Results There was a statistically significant difference in the levels of miR-7, Bcl-2, Bax, and caspase-3 among the groups(P<0.05), and with the increase of EEG abnormalities, the levels of miR-7 and Bcl-2 decreased, the levels of Bax and caspase-3 increased, and the EEG firing index increased. The results of Pearson correlation analysis showed that there were correlations between miR-7, Bcl-2, Bax, and caspase-3 levels and EEG firing index in patients with traumatic epilepsy(P<0.05). The results of multiple linear regression analysis showed that there were correlations between miR-7, Bcl-2, Bax, and caspase-3 levels and EEG abnormalities(P<0.05). Conclusions MiR-7 and apoptotic molecules are correlated with EEG discharge index and EEG abnormalities in patients with traumatic epilepsy. Early EEG examination should be performed. The prognosis should be judged. A scientific and reasonable treatment plan should be made in combination with the patient’s medical history, course of disease and the results of miR-7 and apoptotic molecule detection.
